I am in-network with the following insurance companies:  Blue Cross Blue Shield, Magellan, Cigna, Optum/United Healthcare, Aetna, Baylor Scott and White and Various EAPs.

 

As an in-network provider of the above insurance plans, I will file with your insurance company for services rendered.  You are responsible for any amount the insurance company does not cover.  Payment is due at the start or end of each session.

I accept cash, credit card and tap to pay for any co-pays, deductibles etc.
